🔑Concepts

Reproductive health, as defined by the WHOWHO, is total well-being in all aspects of reproduction, including physical, emotional, behavioral, and social components.

Amniocentesis is a fetal sex determination and disorder test based on the chromosomal pattern in the amniotic fluid. It is often misused for female foeticide, though its primary purpose is detecting genetic disorders like Down syndrome (47,XX+2147, XX+21 or 47,XY+2147, XY+21).

Contraceptive methods are classified into: Natural (Periodic abstinence, Lactational amenorrhea), Barrier (Condoms, Diaphragms), IUDs (CuTCuT, Cu7Cu7, MultiloadMultiload 375375), Oral pills (SaheliSaheli - a non-steroidal once-a-week pill), and Surgical methods (Vasectomy and Tubectomy).

Intrauterine Devices (IUDs) like CuTCuT release copper ions (Cu2+Cu^{2+}) which suppress sperm motility and the fertilising capacity of sperms.

Medical Termination of Pregnancy (MTP) is legal in India (since 19711971) and is considered relatively safe up to the first trimester, i.e., 1212 weeks of pregnancy.

Sexually Transmitted Diseases (STDs) include Hepatitis-BB, HIVHIV, and Genital Herpes, which are currently not fully curable. Others like Syphilis and Gonorrhoea are curable if detected early.

Assisted Reproductive Technologies (ART) include ZIFTZIFT (Zygote Intra-Fallopian Transfer), where the zygote or early embryo up to 88 blastomeres is transferred, and IUTIUT (Intra-Uterine Transfer) for embryos with more than 88 blastomeres.

GIFTGIFT (Gamete Intra-Fallopian Transfer) involves the transfer of an ovum collected from a donor into the fallopian tube of another female who cannot produce one but can provide a suitable environment for fertilization.

ICSIICSI (Intra-Cytoplasmic Sperm Injection) is a specialized procedure to form an embryo in the laboratory in which a sperm is directly injected into the ovum.

💡Examples

Problem 1:

A couple is unable to conceive due to a very low sperm count in the male partner (<20×106< 20 \times 10^6 per mlml). Suggest a suitable ART technique and explain the process.

Solution:

IUIIUI (Intra-Uterine Insemination) or ICSIICSI (Intra-Cytoplasmic Sperm Injection).

Explanation:

In IUIIUI, the semen is collected either from the husband or a healthy donor and is artificially introduced into the vagina or the uterus. If the sperm count is extremely low or motility is zero, ICSIICSI is preferred where a single sperm is injected directly into the cytoplasm of the egg under 400×400\times magnification.

Problem 2:

Identify the chromosomal condition detected via amniocentesis if a fetus has an extra 21st21^{st} chromosome.

Solution:

Down Syndrome (47,XX+2147, XX+21 or 47,XY+2147, XY+21)

Explanation:

Amniocentesis allows the study of the karyotype of fetal cells. An extra copy of chromosome 2121 results in trisomy, leading to Down Syndrome, which is a significant genetic disorder.

Problem 3:

During an IVFIVF procedure, an embryo is developed to the 1616-blastomere stage. Where should this embryo be transferred for further development?

Solution:

Intra-Uterine Transfer (IUTIUT)

Explanation:

As per clinical protocols, embryos with more than 88 blastomeres (>8> 8) are transferred directly into the uterus (IUTIUT), whereas embryos up to 88 blastomeres are transferred into the fallopian tube (ZIFTZIFT).
